What is a Primary Care Network?

A Primary Care Network (PCN) is a group of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations.

How does a Primary Care Network differ from a GP federation?

South Sefton Primary Healthcare is a GP Federation. We are made up of 28 GP practices in south Sefton, working together to deliver patient-focussed services to our residents.

A PCN is a broader collaboration of practices and other health and care partners who work to improve health and care in the area.

We work closely with our local PCN, South Sefton Primary Care Network, to improve the local health and care offer for our residents, provide additional services and support our GP practices.
